Most deal databases report closed sales. Senior Care Intel finds owners before a home is listed, with HUD loan maturities and rates, ownership-change watch, who’s buying, deal news and broker listings alongside.

Each score starts from a base of 35 and adds or subtracts points for each signal found, with the source of each. Possible buyer activity is shown separately and does not add points.

Data and verification
Every signal and contact in Senior Care Intel traces to a named public source. Owner research is checked against a second, independent source wherever one exists, and the result is shown on the record.
| Source | Used for |
|---|---|
| CMS provider and ownership data | Facility profile, owners of record, ownership history, ratings, staffing |
| CMS change-of-ownership filings | Completed and pending ownership changes |
| Medicare cost reports | Revenue, net income, occupancy, payer mix |
| Inspections and penalties | Survey deficiencies, fines, special focus status |
| Federal provider registry (NPPES) | New operator companies registered at a home’s address |
| State licensing rosters | Licensed operators, license changes, assisted living and memory care |
| Secretary of State and county records | LLC officers, registered agents, property ownership |
| HUD Section 232 loans | Insured mortgages, refinancing and reported defaults |
| Published news | Reported transactions, always marked “reportedly” |
Data accuracy
The methodology behind every record, stated plainly. Each figure below comes from the data as loaded in Senior Care Intel, October 2026.
The dataset covers 14,690 skilled nursing facilities listed by CMS, plus assisted living and memory care from state licence lists: NY and PA today, more states added regularly. Ownership, ratings, staffing and inspection history come from CMS public files, including 1.3 million inspection citations since 2017, inspection dates and CMS’s Special Focus list.
Revenue, expenses, margin, per-bed figures, agency staffing cost, occupancy and payer mix for 13,888 homes come from Medicare cost reports filed with CMS. Each figure shows its fiscal year. Cost reports are self-reported and can lag one to two years. Where a home has no report, the field reads “Not reported”. The one derived figure, EBITDAR, is labelled an estimate.
Owner and decision-maker contacts are checked against two independent public sources. A contact earns Verified ×2 only when both sources tie the number to the same person or company. Anything less is labelled as such on the record.
When new operator registrations, state licence changes or published news suggest a home has changed hands, it is flagged as possibly sold, with the buyer and the source, so you do not call an owner who already sold.
Senior Care Intel is built from free public sources, listed above. Each source is cited on the record it supports, so any figure or contact can be checked.
How it compares
Other tools are deal databases (closed sales), facility analytics (no owner contacts) or general property-owner data (not senior-care specific).
| Senior Care Intel | Typical tools | |
|---|---|---|
| Who may sell next | Seller score on every home, with reasons | Sales reported after they close |
| Early signals | New operator companies, filings and public records, often before a sale is announced | Ownership changes once filed |
| Owner contact | Owner or decision maker, each number with its source and status | Facility lists, or owner data not specific to senior care |
| Workflow | Daily call list, call log, follow-ups, deal sheet | Export to a spreadsheet (CSV) |
| Senior care depth | Ratings, staffing, penalties, cost reports, ownership chains | Deep analytics, or none |
| Valuation | Range from public comps and the home’s own numbers | Past deal prices, sold separately |

FAQ
The data, the plans and the trial.
Public records: CMS ownership, quality ratings, staffing, inspections, penalties and change-of-ownership data; Medicare cost reports; HUD Section 232 loans; the federal provider registry (NPPES); state licensing rosters; Secretary of State filings and county property records; and published news, marked “reportedly”. Every flag shows its source.
From public sources such as company websites, state filings, the federal provider registry and news, each with its source link. Where a second, independent source exists, it is checked too. The status shows the result: verified by two sources, single source, conflict, or unverified. Numbers are never guessed.
Every facility gets a score from 0 to 100 built from signals that have come before past sales: ownership tenure, owner size, losses, low occupancy, administrator turnover, agency staffing, HUD loan timing and more. Every reason behind a score is shown. The score, the likely-to-sell list and value estimates come with the With Likely to Sell plans.
You stay in control: Senior Care Intel organizes the research; you make the calls. Calls go out from your own phone, and every outcome is logged on the home’s record.
